Rear shock mount rubber

Hello, just installed new shocks in the rear with new mountings, after a bit of driving, my shock went throught carpet and it took the rubber from the mounting, did my mechanic install it incorrectly?

He messed up somehow...I suspect he forgot a small washer on the shaft. Very easy diy.

common mistake I’ve been posting about for over a decade.
forgot the bottom washer which allows it to press the rubber out of the mounting base.
have shop fix this.
